1. The attached form is four (4) sheets.  Please print out.
2. Write the scouts name and phone number atop each sheet.  The first two sheets are for the scouts PRIMARY choices.  The last two sheets are for the scouts SECONDARY choices. (as marked)
3. Circle in a legible manner the choice for each session.  A selection MUST be made for each hour.  Exceptions must be approved by the Scoutmaster or his designee.
4. Please only signup for MB which have not yet been completed.

UPCOMING CAMPOUT
There is a Campout at Camp Sequoyah for the weekend of April 25th.  The cost for the trip is $15.  Please submit payment for this trip *SEPARATE* from any other payments.  Checks should be made payable to "TROOP 533"
